Things You Need To Know Before Making An Appointment

  • Visit If you choose to see our Nurse Practitioner, new patient visits are comprehensive and will last 60 minutes. This will provide plenty of time to discuss concerns as well as for our providers to collect critical information that will help in finding the root cause of concern.

  • Insurance Our office does not file insurance as we are a small practice and try to keep our cost affordable. If you have insurance, you may contact your provider and inquire about out of network coverage. Some insurance companies will cover a portion of the visit depending on the plan. If you need a superbill, please request that at checkout each time you visit our office. You are able to utilize your insurance for certain services such as lab work if desired, however you are responsible for knowing the coverage of these per your plan.

  • Pricing New patient visits with all of our practitioners are $195. A credit card deposit for the full amount of the 1st visit is required to reserve your new patient appointment.

  • Registering for the Patient Portal We ask that you register for the patient portal prior to your initial visit as this portal is a HIPPA compliant space in which messages can be sent, information about your visits at the office can be accessed, as well as any supplement protocols and medication information is available.

  • Medical History If you have a significant amount of medical history in which you would like sent to our office, you will need to send it to us through your patient portal prior to your visit. All other medical history will be taken at your initial visit.

  • Our Approach As our practice is a blend of traditional and alternative medicine, we often utilize a technique called Nutrition Response Testing in order to carefully determine an appropriate and synergistic protocol for healing. Supplements recommended in the protocol are available at an additional cost.

  • Payment Policies Well Within Family Practice is a “fee for service” facility. We do not accept ANY form of insurance. Our facility is approved for HSA (Health Savings Account) spending allowances toward any service or supplement purchases. Additionally, you may submit your invoice to your insurance company for reimbursement if allowed. We recommend you check with your insurance provider PRIOR to making an appointment if this is an important factor. Pre-payment for the full amount of the first visit is required to reserve your new patient appointment. Thereafter, payment is expected at the time of service. We accept cash, check, and all major credit cards. In the event of a returned check, a returned check fee of $35 will be charged, in addition to the bank fees charged to us.   • Cancellation, Reschedule, Late & No-Show Policy Existing patients who no-show, cancel, or reschedule an appointment within 24 hours of their scheduled appointment time will be charged a cancellation fee of $50. Existing patients who arrive more than 15 minutes late to their scheduled appointment, will be required to reschedule and will be charged a cancellation fee of $50. A credit card is required to reserve your new patient appointment. If you need to re-schedule, we require a minimum of 24 hours notice. New patients who no-show, cancel, or reschedule an appointment less than 24 hours of their scheduled appointment time will forfeit their ENTIRE deposit.
